לאור השימוש הגדל והולך בקנאביס רפואי לטיפול במגוון רחב של מצבים רפואיים ומספרם הרב של המקורות המחתרתיים שכרגע מספקים אותו לפציינטים, באופן טבעי ישנו עניין מיוחד בחקר החוזק, הטוהר, וההרכב הכימי של אספקת הקנאביס הרפואי הזמינה.

למרות שזמינות הקנאביס הרפואי גדלה מיד לאחר שעברה הצעת החוק 215 ועוד יוזמות חקיקה בנושא קנאביס רפואי שהועלו מטעם מדינת קליפורניה, עדיין נותר מחסום בכל הנוגע למחקר המדעי על תכולתו ומרכיביו של הקנאביס.

במאמץ לשפוך אור על נושא מעורפל זה,  הוקם פרויקט מחקרי על ידי קבוצה מטעמנו, הכוללת חוקרים, מגדלים, ונציגי מועדונים של צרכני קנאביס רפואי, עם תמיכה מ-2 ארגונים בקליפורניה, על מנת לנתח דגימות של קנאביס רפואי מספקים וממועדוני מטופלים שונים בקליפורניה.

המאמץ הזה הפך לשיעור בקשיים ובאי הודאות של חקר הקנאביס בחברה בה חופש המחקר התרופתי נחנק ונרמס על ידי בירוקרטיה טוטליטרית יעילה.
עוד בתחילת הפרויקט שלנו כבר נאלצנו להשביתו מפאת חוסר נגישות למעבדות מוסמכות המתמחות בביצוע בדיקות של קנאביס.

מעבדת המחקר המובילה בקליפורניה סירבה לעבוד איתנו מתוך פחד שחוזים שיש למעבדה עם הממשלה יועמדו בסכנה, ושאר המעבדות היו כולן זרות ולכן לא נגישות עבורנו מבחינה חוקית בגלל הגבלות שנאכפות על ידי המנהל לאכיפת חוקי סמים בארה”ב.

בסופו של דבר, היינו מספיק ברי מזל כדי לקבל את שירותיה של מעבדה מורשית מטעם המנהל לאכיפת חוקי סמים, וציוד (מכשיר אנליזה המבוסס על גז) אך עדיין מעבדה זו מעולם לא התנסתה בביצוע אנליזות של קנאביס, ולמעשה תחום עיסוקה המרכזי של המעבדה היה איתור סמים בשתן…

ולכן, בדיקת הדגימות שלנו הפכה לתהליך לימודי עבורנו ועבור המעבדה בד בבד.

מטרתנו המקורית הייתה להשיג אבחון וניתוח רחב של כמה שיותר מתוך שישים הקנבינואידים הנמצאים בטבע, בתקווה לזיהוי הבדלים העשויים לייצר השפעות רפואיות \ טיפוליות שונות של הדגימות השונות.
לאכזבתנו הרבה, המעבדה איתה עבדנו הצליחה להגיע לסטנדרט בדיקה מעבדתי רק עבור שלושת הקנבינואידים הנפוצים ביותר –THC, CBD, ו-CBN

47 דגימות קנאביס שונות נמסרו לנו על ידי יותר מ-6  מועדוני מטופלים וספקים שונים מקליפורניה ועד החוף המזרחי, היו שם 42 דגימות של סנסימיליה (גראס ללא זרעים) , דגימות של חשיש או שרף קנאביס, משקה אחד מבוסס-חלב המכיל קנאביס, וכדור אחד לבליעה המכיל תערובת עלי קנאביס.

לאחר בדיקה בסורק שבמעבדה, נתגלה שריכוז החומר הפעיל ב-42 הדגימות של הסנסימיליה היו בטווח של 12.2 עד 31.6 אחוז THC, עם ממוצע של 19.4 אחוזים.

התוצאות הללו היו גבוהות באופן מפתיע, בהתחשב בכך שריכוז החומר הפעיל במריחואנה בארצות הברית מוערך ב-3 עד 4 אחוזים עם ממוצע גבוה יותר בזנים כגון סנסימיליה – בהם ניתן למצוא כ-10 עד 15 אחוזים.

הריכוז הגבוה ביותר שנרשם היה בדגימה של חשיש, שהגיעה ל68.6 אחוזים.

עם זאת גם דגימה של גראס ממוצע ממקסיקו הכילה ריכוז גבוה באופן מפתיע – 11 אחוזים, פי שתיים מהריכוז לו ציפינו.

ממצאים אלו הטילו לצל כבד של ספק  באשר לדיוק תוצאות הבדיקה, אם כי היה לנו ברור שאנו מתעסקים עם זנים המכילים ריכוזים גבוהים של חומרים פעילים.

בניגוד מוחלט לממצאים אלו, רמות ה-CBD שנצפו היו נמוכות באופן מפתיע, רק ארבע מבין הדגימות נטולות הזרעים הכילו יותר מ-0.3 אחוזים של CBD, בעוד 35 מהן הכילו רק עקבות של החומר כלומר, פחות מ- <0.1%, למרות זאת, בדגימה אחת מצאנו ערך גבוה במיוחד של CBD – 28% (בנוסף ל11.6% (THC.

ובדגימה אחת מצאנו 5.6% CBD יחד עם 13.4% THC, חוץ משתי החריגות הללו ניתן היה לומר בכנות שתוצאות בדיקות ה-CBD היו בהחלט מאכזבות, בעיקר מכוון שקיווינו לגלות הבדלים רבים ומשמעותיים יותר בין הדגימות, שיעידו על אפקט טיפולי שונה לזנים שונים של קנאביס.

בשל ההנחה שה-CBD הוא חומר שמסייע לשליטה בעוויתות שרירים ובהפחתת חרדות ותגובות פאניקה הנגרמות על ידי ה-THC, הנחנו גם כי מטופלים מסוימים יעדיפו זנים המכילים ריכוזים גבוהים יותר של CBD, אך עדיין, נראה היה כי מטופלים מעטים יחסית נחשפים לזני קנאביס המכילים ריכוזים גבוהים יותר של  CBD. לרוע המזל, לא יכולנו להשיג דגימות נוספות של הזנים בעלי ה-CBD הגבוה על מנת לבצע בדיקות נוספות.

ובאשר ל-CBN, רוב הדגימות הראו רק עקבות של החומר, הרמה הגבוהה ביותר שנמצאה היא 1.4% ורק בדגימה אחרת אחת נמצא יותר מאחוז אחד של החומר.

CBN הוא תוצר לוואי של-THC, לכן ציפינו למצוא ריכוזים גבוהים יותר של CBN בדגימות ישנות ומאיכות ירודה, השערה זו אוששה בכך שאחת הדגימות שהכילו מעל אחוז של CBN הייתה בת מעל יותר משנה.

אם כך, רמות ה- CBN הנמוכות בדגימות היא עדות המצביעה על כך שרוב הקנאביס הרפואי הוא טרי ומשתמר טוב.

שאם לא כך היה המצב, שאר התוצאות היו לא-רלוונטיות מכוון שככל הידוע לנו אין ל-CBN שום אפקט רפואי משמעותי.

עוד אכזבה שהתגלתה, היא הכישלון באבחון של יותר מעקבות THC או CBD בנוזל המכונה “חלב האם”.

לאחר מחקר נוסף שבוצע, מסקנת החוקרים במעבדה הייתה כי מיצוי קנבינואידים מתוך נוזלים מבוססי שומן באמצעות הטכניקה המקובלת המשתמשת במתנול הינה בלתי אפשרית.

מהתייעצות נוספת שנערכה עם חוקרים אחרים, מצאנו כי אין דרך ידועה לבידוד THC מתוך נוזלים מבוססי שומן.

לאחר מכן, איתרנו מעבדה ששהצהירה כי בידיה “דרך סודית” לביצוע מיצויים של קנבינואידים מתוך שומן, ובקושי רב, הצלחנו לשכנע את המעבדה לבדוק את “חלב האם”, לאכזבתנו, שוב, נתגלו רק עקבות של THC וCBD. על מנת להיות בטוחים בדיוק הממצאים, אחד מאיתנו בלע דגימה של “חלב האם” שכבר הספיק לשהות כמה חודשים במקפיא, ומצא כי הנוזל חזק באופן מענג, אם כך, השיטה של מעבדה זו נכשלה.

נראה כי עדיין יש לשפר את טכנולוגיות האבחון הקימות של הקנאביס על מנת שניתן יהיה לאבחן בצורה מדוייקת יותר נוזלים המכילים קנאביס על בסיס שומן כגון “חלב האם”, בהאנג (מעין מילקשייק קנאביס-המתרגם) , גהי, ומאפים כמו בראוניס.

רמות ה-THC החריגות בדגימות הסנסימיליה העלו ספקות מטרידים לגבי אמינות תוצאות הבדיקה, מנהל המעבדה הביע ספק לגבי תהליך הכנת הדגימות, ואמר כי הוא שם לב לנטיית השמנים להיפרד משאר הנוזל תוך כדי תהליך המיצוי. בעקבות זאת, הוחלט להגיש את חלק מהדגימות לסבב נוסף של בדיקות, בחרנו 6 דגימות, שכללו את הדגימה עם ריכוז ה- CBD הגבוה במיוחד כבדיקה, הוספנו עוד 2 דגימות חדשות ששיערנו שיהיו בעלות ריכוז THC נמוך: דגימה של עלה מאיכות ירודה, ומעט מהמריחואנה הממשלתית שגודלה עבור NIDA  (המכון הלאומי האמריקני לחקר השימוש בסמים), שידוע לנו שריכוז החומרים בה נע סביב  2.9%-3.9% של THC .

סבב שני – בדיקות קנאביס רפואי

בסבב הבדיקות השני, נתגלה שרמת ה-THC הממוצעת של שבע הדגימות הייתה מעט פחותה  מ-15.1%,  אחוזים, בעוד בסבב הראשון הבדיקות הצביעו על כ 15.1%, בששת הדגימות בעלות ריכוז ה-CBD הנמוך, הריכוז שנמצא היה בין 65% לבין 128% מהערכים שנתגלו בסבב הבדיקות הראשון, בבדיקה שהכילה את ריכוז ה-CBD הגבוה נרשמה ירידת ערכים של כ- 60% – 65%

גם בריכוזי ה-CBD וגם בריכוזי ה-THC, מה שהעלה חשד לגבי לחריגה בדגימה. בדגימת המריחואנה של NIDA  נמצאו ריכוזים של כ-3.9% , הקצה הגבוה של הטווח הצפוי. הדגימה מהאיכות הירודה הניבה תוצאה של 2% . דגימת סנסמיליה אחת רשמה תוצאת שיא של 35% בבדיקה החוזרת.
סיבוב הבדיקות השני לא הצליח להפיג את חוסר הודאות שלנו לגבי דיוק התוצאות, בסך הכול מגמתו הכללית של המידע שנצבר אישרה את חשדותינו, שתוצאות סבב הבדיקות הראשון היו גבוהות מדי באופן כללי.

עם זאת, טווח השינוי הרחב שנצפה בתוצאות בין 2 הבדיקות גם בעבור כל זן בנפרד, ערער את בטחוננו להסיק מסקנה מוצקה כלשהיא.
למרות שהיה נראה הגיוני להסיק שאנחנו עוסקים בקנאביס חזק במיוחד, התוצאות הגבוהות שנמצאו במריחואנה של NIDA רמזו כי גם תוצאות הסבב השני של הבדיקות היו גבוהות מדי.
לאחר כמה חודשים של בלבול, נפלה בחלקנו האפשרות לבדיקה מחודשת של תוצאות הבדיקה שלנו דרך קשר עקיף למעבדה אחרת.

מעבדה זו הייתה מוכרת במומחיותה לבדיקת ריכוזי חומרים פעילים בקנאביס, והיא הייתה אותה מעבדה אשר בדקה את “חלב האם”.
בנוסף ל”חלב האם”, מסרנו עוד 7 דגימות של סנסימיליה, הדגימה שהכילה את ריכוז ה-CBD הגבוה, והחשיש החזק.

סבב שלישי – בדיקות קנאביס רפואי

הריכוזים היו נמוכים באופן אחיד בסבב הבדיקות השלישי מאשר בסבב הראשון, בפרופורציות של בין 25-50%, מה שהוכיח חד משמעית שתוצאותיו של הסבב הראשון היו באופן עקבי בקצה הגבוה של הטווח, עדיין ההריכוז הממוצע בשבע הדגימות של הסנסימיליה היה מרשים – 15.4%, פי ארבעה מהמריחואנה של NIDA.
מכאן, אנו יכולים להסיק בביטחון כי המריחואנה אשר מסופקת כרגע על ידי מועדוני קנאביס מחתרתיים היא בעלת איכות גבוהה מאוד לעומת זו אשר מסופקת על ידי NIDA לשמונת הצרכנים החוקיים.

בגלל רמת הTHC הגבוהה מטופלים צריכים לצרוך רק כמות קטנה מאוד מה”זפת” המזיקה וחסרת התועלת מבחינה רפואית ומעט יותר מהגזים הנמצאים בעשן הקנאביס כדי להשיג את אותה מנה טיפולית יעילה.
מסקנה זאת היא משמעותית מאוד לאור הפרסום האחרון כי הסכנה העיקרית בקנאביס נובעת מעצם העישון שלו.

חוץ מTHC לא יכולנו למצוא נוכחות גדולה של הקנבינואידים האחרים, חוץ מבדגימה אחת או שתים שהיו חריגות.
אם כן, מצאנו עדות קלושה לכך שמטופלים העושים שימוש בזנים שונים של קנאביס כדי לטפל במצבים רפואיים שונים, למרות האפשרות לקיומם של קנבינואידים אחרים.

לסיכום, הניסוי שלנו מראה כי בדיקות מעבדה של תכולת קנבינואידים יכולות לתת תוצאות שונות מאוד מבדיקה לבדיקה וממעבדה למעבדה ולא ניתן לסמוך עליהן בכלל במקרה של “נוזלי קנאביס” מבוססי שומן.
